As physicians, we saw many patients with fragmented health records. These patients often saw multiple doctors for various problems. Sometimes these doctors were even in different cities. We could easily see the challenges patients encountered by not having all of their information with them. Some would have to repeat tests and labs, while others had to spend extra days in the hospital waiting for outside records to come.

As a group, we discussed the challenges patients and physicians face due to the scattering of patient information in today's health system. We envisioned a service that would organize all of a patient's medical information online, making this information readily available and portable. Patients should have the freedom to access their record anytime and anywhere, and they should control which physicians have access to their information.

We teamed up with IT professionals to create a secure and user-friendly web-based program offering these services, while strenuously maintaining privacy. Our healthcare professionals do all the work for patients, so maintenance of records is easy. Until YourMedChart, there was no health record system in which patients could maintain personal health records that were organized in the way doctors use them in clinical practice. Created by physicians, our goal is to both provide access to healthcare records to patients and provide IT solutions to physicians to enhance the patient-physician interaction and improve the delivery of healthcare.
